NURSE PRACTITIONERGeneral PurposeNurse Practitioner will provide geriatric nursing care to older adults by working along with the social worker, Primary Care Physician, and the Clinical department team. The nurse practitio! ner will also offer home-based geriatric assessment and care management for older adults. Nurse practitioner will also engage families and caregivers in the older adults care. Duties and Responsibilities Nurse Practitioner is responsible for the following: Provides geriatric care to older adults receiving health care in an affiliated health care system Offers home-based geriatric assessment and care management for individuals aged 65 or older In collaboration with the social worker, the primary care physician and the interdisciplinary team, the nurse practitioner evaluates an older adults medical and functional status and Works to implement a plan of coordinated care that supports the individuals goals, strengths, and preferences Provides expert, quality and comprehensive geriatric nursing care to vulnerable older adults who are at risk for geriatric complications and functional decline Engaging and educating families and caregivers in the older adults care The NP is respon! sible for: In-home comprehensive geriatric assessment Individu! alized care plan development and interdisciplinary team led by a geriatrician and including a pharmacist and mental health liaison Activation of protocols and team suggestions for care related to geriatric conditions Team meeting with the PCP to review, modify and prioritize the care plan Implementation of the care plan; in collaboration with the PCP and consistent with the patients goals Weekly interdisciplinary team meetings to review care plan implementation Ongoing support team care management supported by an EMR and Web-based tracking system, and providing coordination and continuity of care As an independent practitioner, the nurse practitioner evaluates medical and functional statuses and provides treatment planning and interventions with a high degree of autonomy, exercising independent judgment and initiative in carrying out the nurse practitioner functions and in applying nursing values and ethics to complex situations Detailed Description: Screening/Assessment - ! The nurse practitioner will provide initial and ongoing assessment of an individuals medical, functional, and psychosocial status. The nurse practitioner will obtain relevant health and medical history from the individual and their caregiver if applicable. The assessment will include performing a physical exam appropriate for age and history, comprehensive functional assessment when clinically indicated, conduct preventative screening procedures to identify medical and health risks and needs. Diagnostic tests should be ordered, conducted and interpreted as needed. The nurse practitioner should formulate appropriate differential diagnosis based on the history, physical exam, clinical findings, and diagnostic test findings. The goal of the in-home assessment is to identify medical and health risks and needs while maximizing the health potential of the individual and the family and caregiver. The nurse practitioner will integrate the assessment findings into an overall plan of! care in collaboration with the primary care physician, interdisciplina! ry team, the older adult, and caregivers, whenever possible. Treatment Planning/Goal Setting - The nurse practitioner should update and record any changes in the individuals health status. It is the responsibility of the nurse practitioner, working with the interdisciplinary team and older adult, to develop and implement a comprehensive treatment plan of care based on the assessment. The treatment plan should seek to maximize the health potential and quality of life of the individual. The nurse practitioner should identify the needs and goals of the individual, family, or community as a result of the evaluation of the collected data. The nurse practitioner should make appropriate referrals to physicians, other health professionals and community agencies as needed. The nurse practitioner will prescribe pharmacologic therapies, tests, and services within the confines of their prescriptive authority from the issuing State. Care Plan Development - The nurse practitioner should ! work with the social worker and interdisciplinary team to develop a care plan and communicate that plan to the patient, caregiver, and the primary care provider. The nurse practitioner should monitor for the effectiveness of the care plan and modify the plan as necessary to achieve medical and health goals. Education/Health Promotion - The nurse practitioner will provide relevant education to the individual and caregiver about health promotion, disease management, geriatric conditions, medications, and recommended therapies or interventions. Advocacy - The nurse practitioner will advocate for older adults to ensure they have access to needed care and resources within the community. When appropriate and feasible, the nurse practitioner will educate and encourage the individual and his or her family/caregiver to advocate on his/her own behalf, fostering independence and empowerment. Collaboration - The nurse practitioner will work to create an atmosphere in which nurses build! professional relationships with peers and colleagues in the interdisci! plinary team. The nurse practitioner will serve as an educational resource for staff. Supportive Counseling - The nurse practitioner will provide supportive counseling with the individual and his or her caregiver as needed. Counseling will be offered with the intent of assisting the individual and caregiver in understanding the illness and coping with its impact, addressing end-of-life and bereavement issues, and evaluating the need for more specialized mental health care. Crisis Intervention - The nurse practitioner will assess and intervene in any medical or psychiatric crises presented by the individual and participate in developing crisis management and safety plans to minimize the event of future crises. Non-patient care responsibilities â€" The nurse practitioner will enter all patient/family contacts in the electronic record using appropriate formats and templates. This information will be entered in a complete, confidential, and professional manner to insure informa! tion on the patient is shared with other health care staff. This information may be reviewed by a supervisor or peer reviewer on a regular basis. The nurse practitioner will establish and maintain positive working relationships with employees, volunteers, patients/consumers and other stakeholders within the health care system and outside community agencies. The nurse practitioner will attend all appropriate staff meetings (interdisciplinary team meetings, nursing staff meetings, other committees as appropriate or as assigned) and perform other duties as required to meet the mission of the program or the health care system in general. The nurse practitioner will collaborate in any performance improvement processes and comply with performance measures as required by the health care system. The nurse practitioner will participate in quality improvement review of nursing practice and health care delivery based on research on a periodic basis, including systematic review of reco! rds and treatment plants, helping disseminate findings, and implementin! g plans for improvement. The nurse practitioner will work collaboratively with a physician who will monitor their practice.
NURSE PRACTITIONERQualifications: Graduate from a program accredited by the National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE) Masters or doctoral education concentrating in geriatric/gerontology area of advanced nursing practice Meet registered nurse and nurse practitioner license requirements Certified by American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) Basic CPR Certification In-depth knowledge base and clinical skills related to geriatrics 1-2 years experience with geriatric population preferred Outstanding interpersonal and communication skills Current drivers license Bilingual in Spanish or Chinese a plus
