The Manager of Disease Management/Health Enhancement will serve as a subject matter expert leader, facilitator, and catalyst for improving the clinical and operational performance of multiple functions in the organization's Quality Department using population-based approach to providing improved health and value to our patients. This individual will work on program development and processes that will enable the widespread adoption of health enhancemen! t and chronic condition management programs throughout the ent! erprise. The successful candidate will be knowledgeable in systems improvement and have the ability to design and implement multifaceted projects and facilitate a culture focused on improving population health through the application of various tools aimed at better management of chronic health conditions. This position requires travel to meetings and training throughout the SHCVR as well as SH meetings held in location outside of the SHCVR.
Sutter Gould Medical Foundation (SGMF) is a non-profit, multi-specialty health care organization affiliated with Sutter Health. Based in Modesto, SGMF provides quality medical care, health education and research activities in a five county area. SGMF's medical staff includes family practitioners, internists and pediatricians as well as a range of specialist physicians. Services include imaging, nuclear medicine, a health library, health education programs, occupational medicine, diabetes education, nutrition counseling, inferti! lity counseling and weight management.
Qualifications
Bachelor's Degree in Nursing, Public Health, or Health Administration required; Master's Degree in Public Health, Business Administration, or Hospital/Health Care Administration preferred with knowledge of ambulatory clinic operations. Must have current California RN License( for BSN/MSN). 5-7 years of clinical experience; or any combination of education and experience, providing an equivalent experiential background. Understands medical management principles and practices, financial processes, medical operations and planning (business and strategic), and public relations. Must also be familiar with disease management practices. Skills must include: understanding and using computers and software programs to include work processing, spreadsheets and graphing functions; the ability to analyze and interpret data and prepare concise, accurate reports; must have problem-solving skills as well as the ability ! to coordinate and communicate effectively with diverse others including! , colleagues and physician providers. Must possess: Exceptional interpersonal skills, including the ability to develop and maintain relationships with providers, staff, management, etc. Strong administrative and organizational skills with excellent attention to detail are required. Strong organizational skills, with ability to multi-tasking and work in a very fast paced, dynamic environment with both collaboration and autonomy. Excellent verbal and written communication skills. Strong analytical and critical thinking skills. Ability to prioritize tasks to completion and meet deadlines. Experience translating information and knowledge, developing dashboards, and analyses to drive quality improvement activities. Ability to present complex information in an understandable and compelling manner. Competency on quality improvement methodologies and application. Proven administrative and management ability in complex environments, including supervisory experience. Innovation, flex! ibility, and creativity. Working knowledge of Lean or other continuous improvement methodology preferred. Demonstrated skills as change agent, self-starter and team player. Ability to work in a fast-paced office environment. Prolonged sitting; some bending, stooping and stretching. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard. Requires a normal range of hearing and eyesight to record, prepare and communicate appropriate reports.
