The Health Care Coordinator is responsible for providing telephone intake of the medical, functional, psychological, environmental and social! supports for the stable clients who are potentially at risk. Interventions will be outlined in a care plan to be implemented by the Health Care Coordinator. The Health Care Coordinator provides routine follow-up and monitoring, as well as pertinent patient education, referral to relevant educational opportunities and information sources designed to maintain function, prevent illness and promote wellness.
Sutter Gould Medical Foundation (SGMF)is a non-profit, multi-specialty health care organization affiliated with Sutter Health. Based in Modesto, SGMF provides quality medical care, health education and research activities in a five-county area. SGMF's medical staff includes family practitioners, internists and pediatricians, as well as a range of specialist physicians. Services include imaging, nuclear medicine, a health library, health education programs, occupational medicine, diabetes education, nutrition counseling, infertility counseling, and weight managemen! t.
Qualifications
Bachelor's degree in socia! l work, gerontology or health related field. LVN or BA with previous experience with care coordination and/or the geriatric population. Previous experience working in a medical or social service setting with a geriatric population. Knowledge of available health care and appropriate community resources appropriate for the Medicare Risk population. Knowledge of individual and family development over the life span and the influence of cultural and spiritual values in health care. General knowledge of illnesses, current treatments, and their physical and psychosocial effects on the individual. Knowledge of applicable laws and regulations, government and insurance benefits. Effective oral and written communication. Excellent interpersonal skills and time management skills, ability to work effectively in a fast paced environment with rapidly shifting priorities and competing demands. Ability to identify and refer medically complex cases to the RN Case Manager. Ability to identify! and monitor trends related to risk assessment, client service needs and gaps in service. Ability to identify and refer quality and risk management concerns to appropriate level of the case management organization for corrective action plans and trending. Ability to research and arrange effective linkages with community resources and alternate services for clients and limited insurance benefits or personal resources. Ability to work independently with minimum of direction. Ability to exercise discretion and make independent judgments, seeking review when decisions represent significant departure from established guidelines. Ability to communicate in Spanish is helpful, but not required. PC skills, work processing and data base programs.
