Medical Doctor, Complex Care Physician (Extensivist)
San Jose, CA (in office / field)
** Requires travel to area medical centers, hospitals, SNFs, and member homes (mileage reimbursed). **
Medical Doctor Job Summary: < /b>
- The Medical Doctor, Complex Care Physician is a specific Hospitalist also known as an Extensivist.
- The Medical Doctor is a licensed Physician who provides services to the senior patient population with complex medical conditions or multiple comorbidities.
- The Medical Doctor sees patients across multiple settings.
- The Medical Doctor will see, treat and manage complex patients (high risk, frail, multiple comorbidities) primarily in established area hospitals and also by conducting field visits to members in their homes.
Medical Doctor Background:
- Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree.
- Must hold an active unrestricted license to practice medicine in the state at hire.
- Must be authorized to prescribe controlled substances, meaning a valid DEA certificate.
- Prefer candidates be Board Certified (or eligible) in Internal Medicine or Family Medicine. < li>At least 2-3 years clinical experience practicing medicine, as a Hospitalist, Family Practice, or Internist.
- Any additional experience working for a Payer Organization (Health Plan/IPA/HMO/TPA/etc) is a plus.
- Excellent written and verbal communication skills; and strong critical thinking, deductive reasoning and decision making skills are needed.
- Ability to implement complex programs and to monitor implementation and modifications.
- Must be willing and able to travel to area medical centers, hospitals, SNFs, and member homes (mileage reimbursed).
